When buyers review a pillow, some of them mention neck pain, and each of those mentions counts as positive or negative. That share is a report from people who bought the pillow, not a measurement, and no pillow here is a treatment. A 2021 review of nine high-quality trials found that pillow design did not change sleep quality in people with chronic neck pain, and the effects it did find were small.
Each pick has at least 300 ratings and enough buyers raising neck pain for the share to mean something. Unlike our other lists, the order follows that positive share instead of our overall score, with two places per brand at most. Every pillow that qualifies appears.

1. Coop Sleep Goods Original Adjustable Pillow
Of the 3,951 buyers who mention neck pain, 80% are positive, both the largest count and the highest share on this list. The pillow is adjustable shredded foam, and Coop says an extra bag of fill comes in the box.
Being able to change the height is the point here, and so is being able to send it back: the 100-day trial covers direct purchases only, and the pillow costs $89.
2. Bedsure Bed Pillow
Of the 204 buyers who mention neck pain, 76% are positive, and at $16.50 a pillow it is the cheapest pick here.
It is fixed fiber, soft by the maker's word, so the height in the box is the height you keep, and 64% of the buyers who mention support are negative.
3. Coop Sleep Goods Eden Pillow
Of the 475 buyers who mention neck pain, 76% are positive. Coop says you can add or remove the Eden's gel-infused foam and microfiber fill, and it has a gusseted edge.
It costs $109, more than any pick here but Purple's, and carries the same 100-day direct trial as the Original, useful while you find out whether a new height suits your neck.
4. MyPillow Premium Pillow
Of the 68 buyers who mention neck pain, 75% are positive, the smallest count on the list, so give the share less weight.
It is fixed shredded foam in four firmness levels at $39.98, and 45% of the buyers who mention neck support are negative.
5. Purple Pillow, GelFlex Grid
Of the 139 buyers who mention neck pain, 69% are positive. The GelFlex grid is the only gel grid pillow here, firm by the maker's word and sold in standard size only, at $211.
Buyers raise its weight often, 73% negatively. Purple offers a 30-night trial on direct purchases.
6. Three Geese Goose Feather Pillow, 600 Thread Count
Of the 252 buyers who mention neck pain, 64% are positive. It is the only feather pillow here, medium firm by the maker's word, at $24.99 a pillow.
Feather keeps its height for a limited time: Harvard Health suggests replacing feather pillows every year or so because they collapse.
7. WEEKENDER Ventilated Gel Memory Foam Pillow
Of the 843 buyers who mention neck pain, 56% are positive, the second-largest count on the list and the lowest share.
It is a fixed block of gel memory foam at $29.99, so the height cannot be tuned, and firmness divides buyers.
Reading the shares
What a positive mention tells you, and what it does not
Pool size matters as much as the share. The MyPillow Premium's 75% comes from 68 buyers, so a handful of reviews would move it, while the Weekender's 56% rests on 843 and the Coop's 80% on 3,951. Our guide to reading pillow ratings explains why small pools swing.
A positive mention can mean a buyer woke without the stiffness they had before, or only that the pillow did not make things worse. Buyers also tend to rate in the first weeks. Read the shares as a signal about fit for people who sleep like you, not as evidence about any condition.
What clinicians say
Height and shape over fill
The NHS advice for neck pain is a low, firm pillow, with the head kept level with the body. Mayo Clinic's advice for back sleepers is a pillow that keeps the neck in line with the chest and back. Harvard Health quotes clinicians who put side sleepers on firmer pillows and back and stomach sleepers on thinner ones, as a rule of thumb for each position.
Two 2021 reviews of the research add context. One, covering nine high-quality trials, found that shape and height seemed to matter more for alignment than fill, with small effects overall. The other found no firm answer on the best pillow height for back or side sleepers. A chiropractor writing for Cleveland Clinic adds that a firmer mattress usually calls for a thicker pillow and a softer one for a thinner pillow, and our loft guide turns that into a check you can do at home.
See a clinician
Ongoing neck pain needs more than a new pillow
Ongoing neck pain is a reason to see a clinician, not to keep buying pillows. A clinician can look for causes a pillow cannot reach and tell you whether a change of pillow is worth trying at all. Bring what you already know: how you sleep, how old your pillow is and what you have changed so far.
If you do try a new pillow, pick one you can adjust or return. Coop's 100-day trial applies to direct purchases, and so does Purple's 30-night trial.
Questions
Questions buyers ask
Can a pillow fix your neck pain?
No pillow is a treatment. Across nine trials, a 2021 review saw no change in how people with chronic neck pain slept from one pillow design to another, so the neck-pain shares on these picks are buyer reports, not results. Ongoing neck pain is a reason to see a clinician.
What pillow height should you look for if your neck hurts?
The NHS advice for neck pain is a low, firm pillow that keeps your head level with your body. Researchers have found no single best height for back or side sleepers, so an adjustable pillow, or one you can return, gives you room to find yours.
Why is the Coop Original first on a list about neck pain?
Because of the share and the count together. Of the 3,951 buyers who mention neck pain on the Coop Original, 80% are positive, the highest share and the largest count here. That is a buyer report, not a clinical result.
How much weight should you give a small neck-pain count?
Less than a large one. The MyPillow Premium share comes from 68 buyers, so a few reviews would move it several points, while the Coop's rests on 3,951. Read the count before the share.
